Since our inception in 2007, PPL has been working in partnership with health and social care organisations across the UK to help improve the lives of patients and carers. Our team has been involved in a broad range of projects with both commissioners and providers, including Primary Care Trusts and Clinical Commissioning Groups, Local Authorities, General Practitioners, Voluntary Sector, Independent Sector, NHS Acute and Community Providers. Key outcomes from our recent work include:
Developing "seamless" universal, targeted and specialist services
Improving day-to-day collaboration between health and social care practitioners; designing information sharing systems, protocols and training; establishing "single point of access" arrangements; developing business cases for service integration; and rolling out joint NHS and local authority services and service centres.
Making independent living a reality
Creating intermediate care services that are readily accessible and navigable by patients, service users, carers and staff, reducing hospital admissions and accelerating the process of returning people to their homes.
Developing patient-centred approaches to quality improvement
Developing and implementing quality frameworks based around the national requirement for all NHS providers to produce a Quality Account. In particular, our work has focused on creating systems and processes that put patient outcomes and experience at the heart of quality measurement and improvement planning.
Ensuring access to urgent care
Developing urgent care services and pathways that better meet patients' needs; managing the full commissioning cycle from strategy development through to implementation of new services, including the creation of service specifications and business cases, management of procurement processes and mobilisation project management.
Helping support people at the end of their lives
Working with patients, carers and specialist healthcare services to identify ways in which services can be improved for patients nearing the end of their lives; developing strategies and business cases to support the implementation of new patient pathways.
Saving money without compromising on quality or care
Designing and delivering Cost Improvement Plans from reducing organisation-wide spend and supporting corporate transformation through to front-line, team-based and clinician-led productivity initiatives with measurable cash results.
Understanding and implementing "Outcomes Based Commissioning"
Strategy, programme and project support that helped our clients rank amongst the fastest improving primary care trusts in the country, with a focus on evidence-based approaches to transforming health outcomes and wellbeing amongst the residents they serve.
Preparing boards and organisations for Foundation Trust status
Working with current NHS providers and their stakeholders to ensure that they have the strategies and structures in place to achieve Foundation Trust status, including being well governed and financially viable.
