PPL Shortlisted for “Performance Improvement in the Public Sector” MCA 2016 Award

Leading independent consultancy, PPL has been shortlisted for two prestigious Management Consultancies Association (MCA) 2016 Awards.

PPL is delighted to have been shortlisted for the “Performance Improvement” award for the Three Borough Hospital Discharge Programme, a single new assessment and hospital discharge process developed in partnership with health and social care colleagues across three London boroughs, bringing together patients, social workers and hospital staff from across the patch.

The Three Boroughs, consisting of the City of Westminster, the Royal Borough of Kensington and Chelsea, and Hammersmith and Fulham, represent one of the most affluent regions in the country with 600,000 registered residents and a 1.5 million daytime population including commuters from various part of the country.  Three Borough residents can attend any one of the 16 hospitals across London, are subject to three different adult social care systems and funding mechanisms. As a result, hospital discharge can be a messy business one which is particularly exacerbated in this area by the multiple locality boundaries and extensive choice of hospitals and community care providers.

In partnership with colleagues from the three localities, PPL were brought in in December 2014 to help health and care organisations across the Three Boroughs to implement an integrated and joined-up 7 day hospital discharge service at scale and as quickly as possible, to prepare for upcoming winter pressures.

PPL were able to rapidly deploy a ‘whole system’ programme of change across 9 key organisations to improve the safety and outcomes for people and their families during the transition home from hospital. Key features of the approach include co-production with patients, staff and managers, strong peer and accountability networks, and a rapid, agile and flexible design and deployment method.

Some of the key achievements include the implementation of a proof-of-concept across 8 patient wards and 4 hospital sites, engagement with more than 200 staff and managers across 9 organisations, and the standardisation of over 100 existing processes into 6 clearly defined processes and protocols.

In the last 10 months we have already seen the early results of improved patient outcomes, a reduction in duplication across organisations, a reduction in hospital stay and better use of community support resources. Staff participation and satisfaction has improved, with 100% of staff believing the new model and approach has significantly improved the overall discharge process. Due to the early success of the programme, PPL are continuing to roll-out across the Three Boroughs, and will be scaling up the project to four additional boroughs, covering a population of over 2 million residents over the next year.
